• Heads UP Concussion / Signs and Symptoms, Action Plan Sheet: to be kept with coach at all times when with team

• Heads Up Concussion / A Fact Sheet for Youth Sports Parents: to be given to parents if concussion is suspected

• Team Roster with Emergency Contact and Information Sheet: to be kept with coach at all times when with team
